Tax ID Certification

As an SEC registered transfer agent, we are required to update and certify the tax status of your account maintained at Mountain Share Transfer, LLC.  These rules apply to all the transfer agents in the United States that are registered with the Securities and Exchange Commission (“SEC”).

US Citizens are required to complete and submit an IRS Form W-9.   Non-US citizens are required to submit either a W-8Ben or W-8BEN-E.  Please note the W-8 forms expire after 3 years.  If this form has been previously submitted, it may have expired and require re-submission.
